After reading the Parent Handbook, please login into your Team Unify account and complete the Acknowledgement section. On the "On Deck" App on your phone, the Safe Sport and Parent Handbook Form Acknowledgement is located under "Account Detail" at the very bottom. You will need to click "Edit" in order to add a date using format MM/DD/YY.
